Use the form below to request a new certification card. There is a $20.00 charge for all replacements. Supplying the date and type of class will speed the processing of the request.
PLEASE NOTE: We can only issue replacement cards to those students who have taken their class at or through First Aid Plus or aligned instructors. If you received your original certification from a different source, contact that agency for your replacement.
